A new managed care program - FAMILY CARE - will soon replace traditional county-operated Long Term Care services for frail elders and adults with physical and/or developmental disabilities.
The change will effect people who are receiving or on waiting lists for home & community-based waiver services, their families/informal supports, advocates, service providers and their employees, care managers, program administrators, county and tribal agencies and their employees, county and tribal govenments, health care providers and others throughout northwest Wisconsin.
- On June 2, 2008, the ' Northwest Family Care Collaborative ' of 11 counties became a new 11 county Long Term Care District (Long Term Care District Statute)
- The NW Long Term Care District (NW-LTCD) is a separate unit of government, created by the counties but operating entirely independently of the counties
- 'NorthernBridges' ~ a new, publicly owned Managed Care Organization (MCO) ~ will operate Family Care under the auspices of the NW-LTCD
- In March 2009, the NW-LTC District will begin providing the Family Care Benefit to eligible adults who reside in the 11 county region and choose to enroll as members of NorthernBridges
- Within two years, over 3,000 people will be served and waiting lists will be eliminated
- An 18 member Board of Directors is guiding the NWLTC District and the creation of the new MCO
- A region-wide network of four Aging & Disability Resource Centers (ADRC) is also being developed. ADRCs offer one-stop-shop information / assistance and determine an individual's eligibility for Family Care
- The ADRCs will work hand-in hand with NorthernBridges but will be totally separate, independent entities
